Clinical Social Worker

We are seeking a skilled Clinical Social Worker to join our team at Enlightened, Inc. in Washington, DC.
About the Role:The ideal candidate will be an active and DC Licensed Clinical Social Worker with at least three (3) years of professional practice experience. The position will assist members in achieving their optimal level of health by engaging them and their providers to assess, plan, and establish individual member goals.
Responsibilities:- Assess members to determine care coordination and case management needs.
- Complete comprehensive assessments of environmental, psycho-social, and support needs.
- Identify problems/barriers for care coordination and implement appropriate care management interventions.
- Create a plan of care to assist members in reducing/resolving problems and achieving their optimal level of health.
- Coordinate care and services with the Account manager, Complex RN Case Manager, and member, family members as appropriate, PCP, Specialist, and Facility/Vendor Providers.
- Active and DC Licensed Clinical Social Worker with at least three (3) years of professional practice experience.
- DC Certification as a Clinical Social Worker.
- Bachelor's degree or higher in a health-related field and DC licensure as a Licensed Clinical Social Worker.
- Valid driver's license with car insurance.
