Case Manager RN

Role Overview

Qualifications

The Case Manager RN utilizes advanced nursing expertise to coordinate comprehensive care for a specific patient demographic, facilitating their navigation through the healthcare continuum.

Essential Qualifications:

  • Bachelor of Science in Nursing (BSN) required
  • Minimum of three (3) years of experience in case management or discharge planning
  • Certification in Case Management (CCM) preferred
  • Active Registered Nurse (RN) license in the relevant states
