Registered Nurse (RN) Job Summary:

As a skilled Clinical Care Coordinator, you will be responsible for collecting patient health data, analyzing it to determine nursing diagnoses, and developing individualized care plans. Your attention to detail and ability to analyze complex information will enable you to identify expected outcomes for patients and develop strategies to attain them.

Nursing Care Responsibilities:

* Collects patient health data, age-specific to specialty area, to inform care decisions.

* Analyzes assessment data to determine nursing diagnoses, identifying potential risks and complications.

* Develops and implements care plans to attain expected outcomes, prioritizing interventions based on patient needs.

* Evaluates patient progress, documenting changes in the medical record and communicating with healthcare teams as necessary.

* Administers medication and intravenous solutions safely, adhering to hospital policies and procedures.

* Reports significant patient condition changes to physicians, collaborating to develop effective treatment plans.

* Counsels and educates patients and families on care plans, empowering them to take an active role in their healthcare.

Requirements & Qualifications:

* Bachelor's degree in Nursing or related field required;

* Current Registered Nurse licensure in Massachusetts; American Heart Association BLS certification also needed;

* Minimum two years of Medical/Surgical experience; Critical Care experience a plus;
