In this role, you will play a vital part in assessing, authorizing, and coordinating quality service plans that meet the social and medical needs of our clients.

Key Responsibilities:

  • Conduct comprehensive assessments of program participants to identify their needs and develop personalized care plans.
  • Collaborate with interdisciplinary teams to review and revise care plans, ensuring compliance with funding source regulations and OCES procedures.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
  • Provide education and support to clients and their families, promoting healthy living and independence.
  • Participate in ongoing quality improvement initiatives to enhance the delivery of services.

Requirements:

  • Current MA RN License
  • At least two years' health-related clinical, supervisory, and/or administrative experience
  • Degree in Nursing can be substituted for experience
  • Passion for working with older adults and individuals with disabilities
