Clinical Liaison Specialist

We are seeking a highly skilled and experienced professional to join our team as a Community/Clinical Liaison. This role is responsible for developing and maintaining relationships with healthcare providers, community organizations, and other stakeholders to promote our services and grow our patient census.
Key Responsibilities:- Consult with physicians, nurses, social workers, and other healthcare professionals to establish coordinated care plans.
- Meet with patients, families, and caregivers to discuss home situations and medical/psychosocial factors relevant to care plans.
- Collect and maintain statistical data on referrals and submit reports as required.
- Establish and maintain working relationships with hospitals, healthcare agencies, and community partners to increase engagement.
- Work closely with clinical managers and team members to provide excellent patient continuity and outcomes.
- Clinical or healthcare background strongly preferred.
- Experience in homecare/home health, healthcare, social work, or case management preferred.
- Ability to work independently and be a self-starter.
- Strong communication and interpersonal skills.
