Clinical Liaison

We are seeking a highly skilled Clinical Liaison to develop and establish community relationships with hospitals, healthcare providers, and community partners to grow our patient census.
The ideal candidate will consult and collaborate with physicians, nurses, social workers, and other disciplines to establish a coordinated plan of care.
Key Responsibilities:
- Consult with healthcare professionals to establish a coordinated plan of care
- Meet with patients, families, and caregivers to discuss home situations and medical/psychosocial factors
- Complete referral information, including intake data and plan of care
- Establish and maintain working relationships with healthcare providers and community partners
- Collect and maintain statistical data on referrals and submit regularly
Requirements:
- Clinical or healthcare background strongly preferred
- Experience in homecare/home health, healthcare, social work, or case management preferred
- Ability to work independently and be a self-starter
- Excellent written and oral communication skills
