We're seeking a full-time Clinical Social Worker to join our ambulatory primary care team. As a key member of our team, you'll work closely with clinical and administrative staff to enhance patient care services along the continuum of care.

This role involves identifying high-risk psychosocial factors of patients, helping primary care staff understand their influence on medical care, and assuming care coordination responsibility. You'll provide treatment or intervention as needed, and work with treating outpatient clinicians to ensure effective treatment.

The Key Functions of the Clinical Social Worker are:

  • Advanced care planning

This is a hybrid opportunity, with onsite coverage 4 days per week and remote coverage 1 day per week. We'll provide the necessary technology and training to ensure you're proficient and productive in all job responsibilities, regardless of work location.

Requirements:

  • Master's degree in Social Work
  • 2+ years of experience in a clinical social work setting
  • Strong communication and interpersonal skills
