Clinical Social Worker

We are seeking a full-time Clinical Social Worker to join our ambulatory primary care team at Mass General Brigham. As a key member of our team, you will work closely with clinical and administrative staff to enhance the delivery of patient care services.
Your primary responsibilities will include comprehensive assessment and case management, patient navigation, health promotion, crisis management, self-management education, transitional care, patient and family support, liaison to community services, short-term counseling, and advanced care planning.
You will also provide treatment or intervention as needed, and work with treating outpatient clinicians to ensure effective and focused treatment.
This is a collaborative approach to providing the best care for our patients.

As a hybrid opportunity, this role will entail onsite coverage 4 days per week and remote coverage 1 day per week.
