Key Responsibilities:

  • Employ nursing expertise to deliver safe and effective preventive, therapeutic, and rehabilitative care to assigned patients, ensuring positive outcomes through diligent documentation and observation.
  • Execute independent nursing tasks in accordance with established protocols, as monitored by the Nurse Manager and Director of Nursing.
  • Administer medications with precision, monitoring patient responses and documenting findings in the medical records to prevent adverse outcomes.
  • Facilitate clear communication among patients, staff, and physicians, ensuring comprehensive documentation and feedback within the medical records.
  • Conduct patient admissions and manage narcotics counts.
  • Complete prior authorizations and perform MAR audits to ensure compliance.
  • Assist in fostering effective communication with patients, contributing to the development of individualized care plans and providing thorough shift reports.
  • Collaborate with interdisciplinary team members to coordinate comprehensive patient care, as evidenced by feedback from various departments.
  • Manage incoming calls and inquiries to ensure accurate and timely communication.

Qualifications:

  • Current LPN licensure in the state of MN.
  • Thorough understanding of MN Statutes relevant to the services provided by our organization.
  • CPR/First Aid Certification or ability to obtain within 30 days of hire.
  • Ability to pass a background check.
  • Valid Driver's License with a satisfactory driving record.
  • Proficient in computer applications, including MS Office and Google Suite.
  • Familiarity with Procentive and Electronic Health Record Systems is preferred.
