POSITION SUMMARY
The Care Manager for the Network Initiative within the Homeless Program is responsible for providing advocacy, coordination, and continuity of evidence-based care for individuals and families in need. This role involves a collaborative approach to triage, assessment, planning, and communication with community services.
KEY RESPONSIBILITIES:
- Conduct comprehensive assessments and coordinate community resources tailored to patient needs in accordance with program guidelines.
- Ensure all clinical and administrative documentation is completed within established timelines, maintaining high standards of quality.
- Foster effective communication and maintain collaborative relationships with both internal and external stakeholders.
- Develop service and treatment plans with clearly defined goals and objectives as per program specifications.
- Manage workflow efficiently and maintain organization in line with program requirements.
QUALIFICATIONS:
- A Bachelor’s degree in a Human Services-related field such as Sociology, Psychology, Criminology, Social Work, Behavioral Health, or Mental Health.
- Proficient in reading, analyzing, and interpreting clinical records, procedures, or governmental regulations.
- Strong written and verbal communication skills, capable of providing information and responding to inquiries from managers, patients, service providers, and other stakeholders.
Full-time or as needed.
