The Family Case Manager provides critical support to low-income families in Washington DC, working closely with them to address their health and well-being needs. This role is an excellent opportunity for a dedicated professional to make a meaningful impact in the community.

About Community of HOPE

Community of HOPE is a mission-driven, innovative nonprofit organization that has been serving low-income, underserved, and homeless individuals and families in Washington DC for over 40 years. As a Federally Qualified Health Center, we provide medical, dental, behavioral health, and care coordination services for the whole family at three locations in DC. Our emphasis on maternal and child health includes a midwifery practice and the only free-standing birth center in DC.

Key Responsibilities

  • Maintain a caseload of 15-25 families enrolled in the Healthy Families Home Visiting program
  • Develop HFA Service Plans to address risks identified in the Parent Survey within one month of enrollment
  • Work with families to identify their goals, capturing them on Family Goal Plans and updating them monthly
  • Document CHEERS observations and HFA Reflective Strategies in every visit with participants
  • Obtain immunization and well-child visit information for all children on caseload
  • Cordinate medical care for caregivers in the prenatal and post-partum period, identifying barriers to care and problem-solving to address them
  • Support parents in tracking and scheduling medical appointments for all target children

Requirements

  • High school diploma required; A.A. or B.A in Human Services field preferred
  • Minimum 2 years of experience in a public and/or human services related field
  • Demonstrated cultural competence in communicating with underserved, diverse populations
  • Ability to conduct home visits; valid driver's license and vehicle required
  • Strong interpersonal skills, cross-cultural sensitivity, and organizational abilities

About the Role

This is a full-time position reporting directly to the Home Visiting Supervisor, with a salary range of $62,000 - $65,000 per year, depending on qualifications and experience.
