About the Role

We are hiring a Care Coordinator to work in our South Paris location. This is a 40-hour per week position that involves providing care coordination services and health promotion for clients in the Behavioral Health Home (BHH) program.

Duties and Responsibilities:

  1. Develop and implement plans of care, ensuring effective service coordination and client participation.
  2. Collaborate with healthcare teams to promote access to care and address behavioral-physical health integration.
  3. Communicate clearly and empathetically with clients, families, and professionals to enhance the client experience.
  4. Maintain accurate and timely documentation, meeting regulatory requirements.

Qualifications:

  • An Associate's or Bachelor's Degree in Human Services or a related field.
