An excellent opportunity has arisen for a skilled Nurse Care Manager to join our team at The Opportunity Alliance.
About the Role:We are seeking a highly motivated and experienced Registered Nurse to oversee BHH health outcomes and collaborate with Primary Care Partners. As a Nurse Care Manager, you will play a crucial role in interpreting health data, solving clinical problems, and working with an integrated health and wellness team.
Responsibilities:- Collaborate with behavioral health staff, peer navigators, primary care partners, and MaineCare staff to achieve recovery-oriented services.
- Interpret health data, solve clinical problems, and work with an integrated health and wellness team.
- Provide nursing consultation to BHH Care Coordinators and Peer/Parent Navigators.
- Oversee BHH health outcomes and collaborate with Primary Care Partners.
- Registered Nurse license in Maine, 2+ years of experience.
- Knowledge of mental health, substance use, and related issues.
- Familiarity with recovery-based approaches and health population data.
- Proficiency in Microsoft Office, ECR, and electronic data collection.
- Good driving record and ability to transport clients.
