At Comprehensive Community Action, our mission is to uplift individuals and communities facing challenges related to poverty and social barriers through advocacy, education, and access to essential health and human services.
Position Overview:
The Transitional Housing Support Specialist is responsible for overseeing and providing essential support services to residents within the New Opportunity Home program.
Work Schedule Requirements:
- Positions available include full-time, part-time, and relief roles with rotating shifts, which may include evenings, nights, weekends, and holidays.
- Due to the responsibilities associated with this role, flexibility in scheduling is required, including covering shifts as necessary.
- Staff may be assigned to work at various CCAP locations.
To excel in this role, candidates must effectively manage the following responsibilities, which may be adjusted to meet the needs of the program:
- Ensure the overall wellbeing of residents, focusing on safety, health, hygiene, education, and emotional support.
- Support residents in compliance with their activity plans and contractual guidelines.
- Advocate for residents to facilitate positive outcomes.
- Assist in gathering necessary assessment data, including educational records and evaluations.
- Supervise resident activities and address personal adjustment issues.
- Maintain regular communication with the Program Coordinator and case managers regarding resident activities.
- Implement and oversee daily living program activities.
- Monitor medication administration as required.
- Provide crisis intervention and employ appropriate behavioral management techniques.
- Collaborate with the team to plan, monitor, and evaluate resident programs.
- Assist with transportation for residents to appointments as needed.
- Keep accurate weekly progress notes and logs.
- Enforce house regulations and procedures in line with program philosophy.
- Participate in case conferences and relevant training sessions.
- Engage in community education about CCAP programs.
- Attend agency team meetings and fulfill other assigned duties.
- Understanding of family dynamics and treatment methodologies.
- Strong organizational capabilities.
- Proficient writing skills for documentation and reporting.
- Ability to implement case plans effectively.
- Effective verbal communication skills for case presentations.
- Knowledge of child development principles.
- Ability to work independently and collaboratively.
- Conflict resolution skills and crisis management abilities.
- Commitment to ongoing training and professional development.
- Willingness to engage in self-assessment and goal setting.
- High school diploma, GED, or associate's degree preferred.
- Experience in transitional or supportive housing, particularly with families.
- Familiarity with relevant social service programs.
- Must pass background checks.
- Documented experience working with children and families.
- Valid driver's license and proof of insurance.
- Current TB test and CPR certification.
The physical demands of this position include the ability to sit, talk, and hear regularly, as well as the frequent use of hands and arms. Occasional lifting of up to 20 lbs may be required. The work environment is a shared living facility with designated office space for staff. Employees may encounter exposure to various health risks and must have reliable transportation for community engagement.
