Job Description

We are seeking a Registered Nurse to join our team as a Patient Care Coordinator for our end-stage renal disease (ESRD) program. The ideal candidate will have 1 year of RN experience and be able to work effectively in an outpatient dialysis setting.

As a Patient Care Coordinator, you will be responsible for:

  • Providing patient education and support throughout the treatment process">
  • Collaborating with the healthcare team to develop individualized care plans">
  • Managing patient flow and ensuring timely completion of treatments">
  • Maintaining accurate and up-to-date patient records">

Required Skills and Qualifications

To be successful in this role, you must have:

  • A degree in Nursing from an accredited school">
  • 1 year of RN experience, preferably in an outpatient dialysis setting">
