Hospice and Palliative Medicine Physician Role

At United Hospice, we are seeking a compassionate and skilled Hospice and Palliative Medicine Physician to join our team. As a key member of our interdisciplinary team, you will be responsible for providing high-quality medical care to our patients and their families.

Key Responsibilities:
  • Participate in the evaluation and recertification of patients' terminal prognosis at regular intervals.
  • Collaborate with our interdisciplinary team to develop, implement, and revise patients' plans of care.
  • Complete thorough documentation of all patient care services provided.
  • Provide medical expertise to our admission and patient care staff on issues related to terminal prognosis and pain management.
  • Review patients' medication regimens for potential adverse reactions, inappropriate usage, and duration.
  • Collaborate with attending physicians to address challenges related to terminal prognosis and pain management.
  • Provide home visits based on patients' agreed-upon plans of care and as directed by our Clinical Director.
  • Ensure patients' general medical needs are met, including prescribing medications and signing death certificates as needed.
  • Provide family members with information about patients' medical management and prognosis within HIPAA guidelines.

Requirements:
  • Current licensure to practice medicine in New York State.
  • Board certification in Internal Medicine, Family Practice, or a related specialty with knowledge of palliative medicine and symptom management.
  • Valid driver's license, as required by operational needs.
  • PECOS enrollment.
