Hospice Nurse

The Hospice Nurse - End of Life Care Specialist provides skilled nursing care to patients in a hospice setting. This role requires a strong understanding of end-of-life care and the ability to work collaboratively with other healthcare professionals to provide high-quality patient care.
Responsibilities- Respond to patient and family needs, either in person or telephonically
- Identify and address patient issues, formulating the best clinical response to maintain patient comfort and quality of life
- Initiate and monitor interventions as needed
- Collaborate with other on-call team members, including registered nurses, medical social workers, and physicians
- Educate patients and families on end-of-life care and support
- Document patient care and charting
- Ensure necessary medical equipment, supplies, and services are available
- Graduate of an accredited school of nursing and current Registered Nurse (RN) licensure in the State of Michigan
- Two years of recent clinical experience, with homecare or hospice experience preferred
- Sound critical thinking and clinical judgment
- Knowledge and/or experience in pain symptom management associated with end-of-life care
- Reliable transportation with a valid state driver's license and current auto insurance
