Job Title: Registered Nurse - Hospice Care Specialist

United Hospice is seeking a compassionate and skilled Registered Nurse to join our hospice team in Orange County. As a Registered Nurse - Hospice Care Specialist, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to deliver exceptional care to our patients and their families.

Responsibilities:
  • Provide direct care to patients utilizing accepted standards of nursing practice and consistent with facility policies and procedures.
  • Administer medications and treatments according to established policy and procedure and document all interventions accurately.
  • Perform thorough and accurate patient assessments and provide appropriate follow-up and documentation of all findings and interventions.
  • Conduct the admission process for all new patients.
  • Participate in hospice-sponsored in-service training as required.
  • Generate the initial plan of care and perform ongoing assessments and care plan updates in collaboration with the Interdisciplinary Team.
  • Perform ongoing symptom assessments of patient and family needs and institute the appropriate interventions.
  • Maintain proficiency in working with technical equipment such as feeding pumps, medication pumps, suction machines, etc.
  • Maintain proficiency in managing intravenous access devices including intravenous medication administration.
  • Document in the electronic medical records system accurately; complete the entries timely to reflect the patient's condition consistent with the plan of care.
  • Collaborate and coordinate effectively with clinical and support staff to conduct the daily care of the patient.
  • Inform the physician and other staff of changes in patient needs and outcome of interventions.
  • Document assessments, interventions, patient responses to treatments, and all communications/conferences/IDT meeting, care plans and discharges.
  • Accept other responsibilities as assigned.
Requirements:
  • Valid and current NYS Registered Nurse license is required; BSN preferred.
  • Two years recent clinical nursing experience with strong clinical skills including one year recent Med-Surg. experience required.
  • Complies with accepted nursing professional standards and practices.
  • Commits to take and pass the hospice and palliative care nursing certification exam (CHPN) within three years of hire if not currently certified.
  • Commitment to the Hospice Philosophy of Care.
  • Ability to relate to a diverse population and workforce in a personable and effective manner.
  • Able to communicate effectively, while sustaining a caring, kind and professional disposition.
  • Must have excellent written and verbal communication skills.
  • Proficient in MS Office Suite, EMRs and ability to learn new software programs.
  • Hospice admission and end-of-life care experience preferred.
  • Must have daily access to a vehicle, valid driver's license/acceptable driving record and auto insurance to travel to patient locations.
  • Flexible schedule with weekend rotation as needed.
