We are seeking a skilled and compassionate Nurse Practitioner to join our team at The Elizabeth Hospice. As a Hospice Nurse Practitioner, you will provide high-quality care to patients and their families, working collaboratively with our interdisciplinary team to ensure comprehensive support.
Responsibilities:- Provide patient-centered care, assessing and evaluating patient needs, and developing individualized plans of care.
- Collaborate with the interdisciplinary team, including physicians, nurses, social workers, and chaplains, to ensure seamless care coordination.
- Conduct home visits, assessing patient needs, and providing education and support to patients and families.
- Document patient information, progress, and care plans in our electronic medical record system.
- Participate in medical staff meetings, nurse practitioner meetings, and other meetings as directed by the Chief Medical Officer.
- Current licensure as a Registered Nurse in the State of California.
- Current licensure as a Nurse Practitioner in the State of California.
- DEA Certificate.
- Valid National Provider Identification (NPI) Number.
- Master of Science Degree in Nursing (MSN) from a recognized university.
- National Certification as a Nurse Practitioner (AANP or ANCC).
- Advanced Certification as a Hospice and Palliative Care Nurse (ACHPN) preferred.
- Minimum of three (3) years of nursing experience in a hospice or palliative care setting.
