About the Job

We are seeking a highly skilled Hospice Nurse Practitioner to join our team at Reliance Hospice and Palliative Care in the San Fernando Valley. As a key member of our interdisciplinary team, you will provide comprehensive hospice services to patients and their families, working closely with our physicians and other healthcare professionals.

Responsibilities
  • Evaluate and assess patient health history, health promotion, and medical conditions
  • Develop and direct plans of care, prescribing medications and treatments as needed
  • Collaborate with patients, families, and other healthcare practitioners to coordinate care
  • Visit patients, complete physical assessments, and evaluate hospice eligibility
  • Build on community resources to plan and direct services to meet individual and family needs
Qualifications
  • Graduation from an accredited School of Nursing
  • Completion of an accredited Nurse Practitioner Program
  • Current nursing licensure in California and CPR certification
  • Minimum 1 year of Nurse Practitioner experience and 1 year of hospice or palliative care experience
  • Accreditation from an approved certifying body for advanced practice nursing (CHPN) preferred

Education and Experience
  • Master's degree required
  • Hospice & palliative medicine experience preferred
  • Language: Spanish preferred
  • License/Certification: Certified Nurse Practitioner required
