Job Title: Per Diem Hospice Nurse Practitioner

Job Summary:

The Elizabeth Hospice is seeking a skilled Per Diem Hospice Nurse Practitioner to provide quality care to patients across the lifespan. As a member of our interdisciplinary team, you will work closely with patients, families, and community physicians to develop and implement individualized care plans.

Responsibilities:

  • Visit patients within a short period of time following admission and as medically necessary afterwards.
  • Ensure documentation is entered into the electronic record within expected guidelines.
  • Coordinate care with the interdisciplinary team.
  • Communicate all changes to the IDT and community physician(s).
  • Seek medical direction for assigned patients as required.
  • Utilize critical thinking to interpret multiple, sometimes conflicting patient data.
  • Make clinical judgments based upon a grasp of the "whole picture" for patient diagnoses, managing changes of condition with appropriate levels of care.
  • Implement and maintain established hospice policies and procedures as well as Standards of Care and regulatory requirements.
  • Orient, instruct, and train assigned personnel.
  • Work with the Education Program Manager to educate staff regarding patient care.
  • When necessary, share in providing 24 hours, seven days a week coverage to patients and families.
  • Available rotating weekends to make patient visits as needed.
  • Attend medical staff meetings, nurse practitioner meetings, and other meetings as directed by the CMO.
  • Counsel patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients.
  • Document comprehensive medical history in patient's electronic record.

Qualifications:

  • Licensed as a Registered Nurse by the State of California.
  • Licensed as a Nurse Practitioner by the State of California.
  • Drug Enforcement Administration (DEA) Certificate.
  • Valid National Provider Identification (NPI) Number.
  • Master of Science Degree in Nursing (MSN) from a recognized university.
  • National Certification as a Nurse Practitioner.
  • CPR/BLS Card.
  • Advanced Certification as a Hospice and Palliative Care Nurse (ACHPN) preferred.
  • Minimum of three (3) years nursing experience in a hospice or palliative care setting.
  • Computer skills, including word processing, use of adding machine, calculator, fax, photocopier.
  • Bi-lingual (Spanish) preferred.
  • Current California driver's license and proof of insurance required.
