At Methodist Le Bonheur Healthcare, we are seeking a skilled Clinical Social Worker to join our team. As a Clinical Social Worker, you will play a vital role in coordinating patient-centered care and service delivery to facilitate optimal transitions and progression in care.
Key Responsibilities:- Coordinate patient-centered care and service delivery to facilitate optimal transitions and progression in care.
- Collaborate with patient, caregiver, physician, and other members of the healthcare management team to ensure seamless care coordination.
- Develop and implement effective transition plans to promote efficient utilization of resources and optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare team members to ensure timely and accurate information exchange.
- Monitor and evaluate the effectiveness of care coordination and transition plans to identify areas for improvement.
- Master's Degree in Social Work (MSW) from an accredited institution.
- 5-7 years of licensed social work experience, with 2 years inpatient/acute care setting.
- Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW) certification.
- ACM/CCM certification preferred.
- Excellent interpersonal and communication skills, with ability to work collaboratively within an integrated interdisciplinary team.
- Comprehensive knowledge of community resources and ability to gather relevant data to inform care coordination decisions.
- Ability to use a variety of social work interventions and adapt to changing patient needs.
- Strong analytical and problem-solving skills, with ability to prioritize tasks and manage multiple projects simultaneously.
