Job Summary

The Social Services Case Manager will be responsible for providing patient-centered care and support services to individuals and families affected by illness or injury.

As a key member of our interdisciplinary team, you will work closely with healthcare professionals to develop and implement comprehensive care plans that meet the needs of our patients.

Responsibilities
  • Provide emotional support and counseling to patients and their families

Requirements

To be successful in this role, you will need:
