Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Minimum Data Set (MDS) Coordinator RN at our Rio Rancho Center in Rio Rancho, NM. As an MDS Coordinator RN, you will play a critical role in ensuring the accurate and timely completion of MDS assessments, which are essential for patient care and reimbursement.

Key Responsibilities
  • Participate in the Patient Assessment Process by completing MDS items and Care Area Assessments (CAAs) in accordance with Federal and State regulations.
  • Assist in the development of individualized patient care plans, identifying patient needs and projected outcomes.
  • Contribute to the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, following RAI guidelines for completion, computer encoding, and electronic transmission of the MDS.
  • Identify changes in patient condition and coordinate scheduling and completion of indicated assessments in conjunction with the interdisciplinary team.
  • Assist with Activities of Daily Living training for new hires and ongoing continuing education for existing staff.
  • Attend educational programs to maintain knowledge of current RAI/Prospective Payment System regulations.
Requirements
  • Must be a Registered Nurse currently licensed by the state and in good standing.
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility is required.
  • Prior experience completing MDSs and Care Plans is required.
  • Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures and laws, regulations, and guidelines governing long-term care.
