Job Summary

We are seeking a skilled RN Case Manager to join our team at the University of New Mexico. As a key member of our healthcare team, you will be responsible for coordinating patient care, managing treatment plans, and ensuring quality outcomes within established time frames.

Key Responsibilities
  • Ensure patient safety and medication administration
  • Conduct patient assessments and develop comprehensive care plans
  • Participate in quality improvement activities and continuous education
  • Collaborate with interdisciplinary teams and community health agencies
  • Document patient progress and maintain communication with healthcare team members
Requirements
  • Program Graduate with a Bachelor's degree in Nursing. Nationally Accredited Nursing Graduate preferred.
  • 2 years of directly related experience preferred.
  • RN in NM or as allowed by reciprocal agreement, CPR for Healthcare/BLS certification.
