Role Overview: Under the supervision of healthcare providers (MD, DO, DPM, NP, or PA) or RNs, the LVN/LPN is responsible for delivering clinical support and professional nursing care to patients, adhering to established nursing standards and procedures sanctioned by the relevant state board of nursing. The LVN/LPN is an integral part of the healthcare team, assessing patient needs, contributing to care plans, and executing prescribed interventions.
Key Responsibilities:
- Conducts nursing duties within the scope of a Licensed Vocational/Practical Nurse, including procedures, injections, medication administration, EKGs, and phlebotomy as per state regulations.
- Implements educational plans for patients and their families addressing common health issues and learning needs.
- Coordinates patient care as directed by healthcare providers and RNs, managing referrals, transitions, medications, procedures, and follow-ups.
- Documents patient complaints and prepares them for examination according to established protocols.
- Collects pertinent information regarding patient symptoms and communicates effectively with the provider.
- Performs focused nursing assessments, evaluates patient responses to treatment, and communicates findings to the provider.
- Maintains accurate and timely records of patient assessments and care in medical documentation.
- Reviews patient records and relevant information prior to provider consultations, reporting significant details to the provider or RN.
- Ensures a safe clinical environment in compliance with OSHA standards.
- Upholds patient confidentiality in accordance with HIPAA regulations, treating all patients with respect and dignity.
- Organizes and maintains examination and treatment areas, ensuring cleanliness and proper stocking of supplies.
- Adheres to infection control protocols and ensures proper medication management.
- Fulfills laboratory management responsibilities and collects lab samples with appropriate labeling.
- Handles medication refill requests in line with established protocols.
- If certified in IV therapy, initiates peripheral lines, monitors flow rates, and manages IV site care.
- Performs additional duties as assigned.
Qualifications:
- High School diploma or GED.
- Current and unrestricted State Vocational/Practical Nursing license.
- Current BLS certification.
- IV certification if required by the clinic.
- Knowledge of medical terminology and office procedures.
- Proficient in using electronic medical records.
- Strong communication skills, both verbal and written.
- Ability to remain calm and effective in emergencies.
- Skilled in performing medical procedures, administering injections, and conducting phlebotomy.
- Ability to foster effective working relationships with providers, staff, patients, and the community.
- Demonstrated customer service skills.
- Compliance with Tuberculosis screening and proof of immunity to certain diseases.
Preferred Qualifications:
- IV certification.
- Two or more years of experience in a medical environment.
- Familiarity with ICD-10 and CPT coding.
Physical and Mental Requirements:
- Ability to lift up to 25 pounds.
- Ability to push or pull heavy objects with up to 25 pounds of force.
- Fine motor skills for operating equipment.
- Ability to comprehend instructions both verbally and in writing.
- Logical reasoning skills for problem-solving.
- Occasional exposure to communicable diseases or bodily fluids.
