Key Responsibilities- Collaborate with Primary Care Physicians (PCPs) to develop and implement integrated care plans that address the physical and emotional health needs of patients.
- Provide behavioral health services to patients, including assessment, diagnosis, and treatment of mental health conditions.
- Work closely with patients and their families to develop and implement behavioral treatment plans that improve overall functioning and quality of life.
- Participate in multidisciplinary teams to coordinate care and ensure seamless transitions between healthcare settings.

- Master's degree in a relevant field, such as psychology, social work, or counseling.
