The RN Care Manager, Per Diem is responsible for overseeing the entire care process, optimizing resource use, and planning discharges for specific patient groups throughout their care journey. This role involves collaboration with healthcare providers, social workers, and interdisciplinary teams to ensure the achievement of optimal patient outcomes, quality standards, and financial efficiency.

KEY RESPONSIBILITIES:

  • Evaluates the patient's requirements, objectives, and obstacles in relation to their medical condition, psychosocial context, and financial resources.
  • Partners with the multidisciplinary team to strategize, execute, and assess the patient's care plan through various methods, including rounding.
  • Works closely with the patient, family, or other caregivers, and the multidisciplinary team to formulate a discharge plan that aligns with the patient's needs and aspirations. Continuously reassesses and adjusts the discharge plan as new information arises or goals evolve.
  • Actively considers options such as palliative care, home health services, and other resources to maintain the patient's health in an outpatient setting, thereby reducing the likelihood of readmissions.
  • Collaborates effectively with other care management professionals, including but not limited to: social workers, assistants, coordinators, utilization management personnel, and leadership.
  • Ensures clear communication among all members of the treatment team.
  • Manages the duration of stay by proactively identifying and addressing challenges and barriers to care and successful discharge.
  • Conducts risk assessments using available tools and implements discharge strategies accordingly to minimize readmission risks.
  • Identifies patients with unplanned readmissions and performs root cause analyses.
  • Coordinates with Outpatient Care Managers to identify patients for handover and follow-up after discharge.

PHYSICAL AND SENSORY REQUIREMENTS:

Ability to sit for extended periods, walk on various surfaces for up to five hours daily, and navigate stairs. Must be capable of driving and frequently handling objects. Occasionally required to lift, carry, push, or pull items weighing up to 75 pounds. May involve stooping, bending, squatting, kneeling, and reaching above shoulder level. Must possess auditory capabilities for normal conversations and various frequencies, as well as tactile skills for telephone and computer use.

EDUCATION:

Registered Nurse with a current license to practice in Pennsylvania or pursuing licensure through reciprocity. New Jersey RN licensure is required for specific campuses. An advanced degree is preferred, with a BSN being ideal; MSN and/or Case Management Certification are also preferred.

TRAINING AND EXPERIENCE:

Extensive clinical expertise with 3-5 years of experience in acute care settings. Previous experience in care management is advantageous. Familiarity with InterQual/Milliman criteria is preferred. Basic computer proficiency is necessary, along with the ability to foster and maintain collaborative and effective working relationships.
