Case Manager

Suncoast Center is seeking a compassionate and dedicated Case Manager to support and advocate for our clients. As a key member of our Clinical Care Team, you will coordinate with clients, family members, and community supports to ensure client needs are met and all involved in the client's treatment work together.
The successful candidate will have a bachelor's degree in social work, psychology, or a related human services field, and at least one year of experience in serving clients with mental health and/or substance use needs. You will be responsible for developing and implementing individualized treatment plans, providing direct clinical services, and linking clients with necessary resources.

Key Responsibilities:
- Develop and implement individualized treatment plans
- Provide direct clinical services, including crisis intervention and on-call support
- Link clients with necessary resources, including medical and dental services, housing, and vocational assistance
- Coordinate with clients, family members, and community supports to ensure client needs are met
- Monitor client progress and adjust treatment plans as needed
Requirements:
- Bachelor's degree in social work, psychology, or a related human services field
- At least one year of experience in serving clients with mental health and/or substance use needs
- Completion of AHCA-approved targeted case management training within 3 months of hire
- Certification through the Florida Certification Board (FCB) within 6 months of hire

Job Requirements:
- Must be 21 years of age
- Must have a negative drug screening prior to hire and throughout employment
- Must pass a Criminal Background Check, local checks, and DCF Level II Background Screening prior to hire and annually
- Must have a valid Florida Driver's License and pass motor vehicle checks prior to hire and annually with an acceptable driving record maintained throughout employment
