Clinical Care Coordinator Lead

**Job Description**The Clinical Care Coordinator Lead will be responsible for providing clinical oversight and case management services to clients, as well as orienting and evaluating caregivers to ensure they have the necessary skills and competency to provide quality care. This role requires strong communication and organizational skills, as well as the ability to work independently and as part of a team.
**Key Responsibilities**- Provide clinical oversight and case management services to clients
- Orient and evaluate caregivers to ensure they have the necessary skills and competency to provide quality care
- Collaborate with office teams to review documentation and ensure compliance with regulations
- Develop and implement individualized plans of care for each client, incorporating analysis of data and current scientific findings
- Provide interventions to achieve realistic client outcomes and report changes to update the plan of care
- Masters or Bachelors degree in a healthcare-related field
- Excellent written and verbal communication skills
- 1-2 years supervisory or management experience
- Proficiency with computers
The salary for this role is estimated to be around $70,000 per year, based on industry standards and the location of the position.
