We are currently seeking an experienced Hospice Care Coordinator to join our team.

Job Summary:

  • Deliver hospice education activities to healthcare institutions and physician offices.
  • Coordinate patient services to ensure seamless transitions between care settings.
  • Promote community awareness of hospice services through educational activities.
  • Foster strong relationships with healthcare institutions and physicians to improve care coordination.
  • Interact with clinical leadership to drive quality improvement initiatives.

Requirements:

  • A bachelor's degree or equivalent in a related field; two years' experience in home health/hospice, hospitals, or social services preferred.
  • Knowledge of Medicare/Medicaid hospice regulations beneficial.
  • Demonstrate ability to coordinate patient transfers, ensuring uninterrupted medical care.
  • Strong knowledge of hospice services, federal, state, and ACHC guidelines required.
  • Excellent communication and interpersonal skills, with proficiency in written and oral communication.
  • Ability to travel as necessary.
