Nurse Practitioner

We are seeking a skilled and compassionate School RN to join our team at Northwell Health. As a School RN, you will play a vital role in providing high-quality patient care and education to students and families in our community.
Responsibilities- Assessment and Care Planning: Conduct thorough assessments of patients' physiologic and psychosocial health status, develop individualized care plans, and implement evidence-based interventions to achieve optimal patient outcomes.
- Collaboration and Communication: Work effectively with physicians, nurses, and other healthcare professionals to coordinate patient care, provide education, and support families in making informed decisions about their care.
- Medication Administration and Education: Safely administer medications, provide accurate information about medication use, and educate patients and families about potential side effects and interactions.
- Discharge Planning and Education: Develop and implement discharge plans, provide education to patients and families about post-hospital care, and ensure a smooth transition to community-based care.
- Quality Improvement and Patient Safety: Participate in quality improvement initiatives, identify areas for improvement, and implement changes to enhance patient safety and satisfaction.
- Graduate from an accredited School of Nursing with an Associate's or Bachelor's Degree in Nursing.
- Minimum 1-2 years of professional nursing experience in a related specialty.
- Current license to practice as a Registered Professional Nurse in New York State.
- BLS certification and specialized certifications as needed.
