About this role:

The Agency on Aging of South Central CT is seeking a highly motivated and caring Compassionate Care Coordinator to join our team in North Haven, CT.

This challenging position involves ensuring the well-being of our older adult clients in a home care setting. As a key member of our care team, you will be responsible for developing and implementing personalized care plans that meet the unique needs and preferences of our clients.

Key responsibilities include providing exceptional social services, including discharge planning, mental health support, and long-term care management. You will work closely with our clients and their families to ensure that their needs are met, and collaborate with healthcare providers and other members of the care team to ensure continuity of care.

To succeed in this role, you will need excellent communication and interpersonal skills, as well as patience and compassion for older adults. A Bachelor's degree in Human Services or a related field is required, along with 2+ years of experience in case management or a related field.

A competitive salary of $47,500 per year, plus benefits, makes this an attractive opportunity for the right candidate.
