Job Description

This Clinical Nurse II will be responsible for assessing, coordinating, planning, directing, implementing, and managing the needs of assigned patients throughout their episode of care. You will work closely with a multidisciplinary team to ensure that each patient receives comprehensive and individualized care.

Key Responsibilities:
  • Assessing patients' physical, emotional, and spiritual needs
  • Developing and implementing individualized care plans
  • Providing education and support to patients and their families
  • Maintaining accurate and timely records of patient care
  • Collaborating with other healthcare professionals to ensure seamless care transitions
Required Skills and Qualifications

To succeed in this role, you will need:

  • A Bachelor's degree in Nursing (BSN) or related field
  • Current licensure as a Registered Professional Nurse (RPN) in New York State
  • At least 1-2 years of experience in a hospital setting, preferably in medical-surgical nursing
  • Excellent communication and interpersonal skills
  • Able to work effectively in a fast-paced environment
