About the Role

CarePartners is seeking a highly skilled and compassionate Home Care Clinical Coordinator to join our PACE program team. As a key member of our interdisciplinary team, you will play a vital role in ensuring the delivery of high-quality, patient-centered care to our participants.

Key Responsibilities
  • Coordinate the day-to-day operations of in-home services for our PACE participants and families.
  • Act as a liaison between the contracted home health agency and the PACE program, coordinating homecare services, including nursing, home health aides, and homecare schedulers.
  • Utilize nursing skills to assess participants in their home environment, coordinate plans of care, and provide treatments and health education as needed.
  • Participate in interdisciplinary team meetings to ensure compliance with competency requirements, maintain accurate medical records, and provide thorough documentation.
  • Develop and implement health and wellness plans in participants' homes, utilizing telehealth, assistive technology, and community resources.
Requirements
  • Current licensure as a registered nurse with the North Carolina Board of Nursing.
  • Bachelor of Science in Nursing degree (Associate Degree or Diploma in nursing with agreement to obtain BSN within 6 years of hire date).
  • Minimum 2 years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting.
  • Minimum 1 year of experience working with a frail or elderly population.
  • Willingness to work in home environments with diverse values and standards.
Preferred Qualifications
  • Computer literacy with EMR and familiarity with telehealth and video consultations.
  • Nursing experience in a community-based setting.
  • ACTT RN or Mental Health experience.
