Patient Care Coordinator

We are seeking a highly skilled Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a critical role in connecting people with the care, pharmacy benefits, data, and resources they need to feel their best.
Key Responsibilities:
- Receives incoming telephone calls in a prompt and courteous manner, working with multiple phone lines and providing telephonic patient outreach and patient experience calls.
- Consistently exhibits behavior and communication skills that demonstrate commitment to superior customer service, including quality and care and concern with each and every internal and external customer.
- Represents the Company in a professional manner, following all Company policies and procedures.
- Uses, protects, and discloses patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
- Responsible for accurately and professionally handling all front desk operations, including answering phones, greeting patients, scheduling of all appointments, EOD Billing, addressing routine inquiries, working daily reports, and forwarding non-routine requests to appropriate personnel.
- Performs clerical duties as directed.
- Coordinates patient care as directed by physicians, company standards and policies.
- Supports and follows Standard Delegation of Orders (SDO).
- Building referrals in EMR systems.
- Updates and maintains ECW with all patient changes.
- Manages medical records (maintains, files/scans, prepares for schedule).
- Ensures all correspondence is scanned and/or filed in timely manner.
- Processes requests for medical records release and maintains appropriate logs, etc.
Requirements:
- High School Diploma/GED (or higher) OR 4+ years of customer service experience.
- 1+ years of data entry and customer service experience with strong communication skills both in person and via phone.
- 1+ year of experience/working knowledge of medical office procedures and medical terminology.
- Intermediate level of proficiency with Microsoft Office applications to include Excel, Word, and Outlook.
Preferred Qualifications:
- Experience in a physician's office, clinical or hospital setting.
- Ability to display excellent telephone/switchboard etiquette.
- Ability to interact and verbally communicate effectively with patients and all levels of personnel in a professional, courteous and effective manner with excellent customer service skills.
- Ability to multi-task in a high paced environment with good organizational skills.
- Ability to follow written and oral instructions and to work with general guidance.
- Demonstrated clerical skills (typing, filing, telephone courtesy).
- Bilingual (English/Spanish) language proficiency.
