Job Summary

The Social Work Lead will supervise the Inpatient and Emergency Department Social Work team at all NCH campuses. The successful candidate will possess excellent communication, interpersonal, and problem-solving abilities.

Promote patient/family-focused care as the highest priority and strive to meet customer expectations of high-quality services. The Social Work Lead will also play a key role in program development, staff training, and community outreach.

Essential Duties and Responsibilities
  1. Support the Director of Case Management and Social Work Manager in implementing and enforcing NCH and departmental policies and procedures.
  2. Supervise, orient, and mentor a team of social workers.
  3. Complete psychosocial assessments, mental health counseling, crisis intervention, financial counseling, discharge planning, and link patients to community resources.
  4. Provide patients/families with resources to meet their medically related social and emotional needs.
  5. Ensure safe transition of patients from one care environment to another.
  6. Promote patient/family involvement and choice in discharge planning.
  7. Advanced experience in guardianship process.
  8. Possess extensive knowledge of PASAAR process for extended care facility placement.
  9. Serve as resource for Baker Act and Marchman Act.
  10. Foster a collaborative and supportive team environment, promoting professional development and best practices.
  11. Oversee the delivery of social work services, ensuring patients receive appropriate, timely support.
  12. Provide direct support to complex cases, including guardianship, unfunded, and undocumented patients.
  13. Ensure compliance with legal and ethical standards in all patient and family interactions.
  14. Collaborate with management team to design and implement social work programs and initiatives.
  15. Identify community needs and develop strategies to address gaps in services.
  16. Evaluate program effectiveness and make recommendations for improvements.
  17. Develop and facilitate training programs for social workers to enhance their skills and knowledge.
  18. Stay informed of evidence-based best practices in social work and collaborate with leadership team on new approaches into the team's work.
  19. Ability to work collaboratively with multidisciplinary teams and community partners.
  20. Assist in staffing, as needed.
Education, Experience, and Qualifications

Minimum of Master's degree in social work (MSW) or a related field from an accredited institution.

Minimum of 2 years in a supervisory/leadership role.

Licensed Clinical Social Worker (LCSW) or equivalent certification preferred.

Case Management Certification Preferred.

Experience working with diverse populations and in various social work settings.

Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
