We are seeking a highly skilled and experienced RN Case Management Team Lead to oversee daily operations and supervision of the NCH RN Inpatient and Emergency Department Case Management Team at all NCH campuses. As a key member of our healthcare team, you will promote patient/family focused care as the highest priority and strive to meet customer expectations for quality and patient satisfaction.
Key Responsibilities- Implement organizational case management strategies with a focus on patient experience.
- Ensure compliance with regulatory standards.
- Train and mentor RN case managers.
- Complete personnel functions including staffing, performance reviews, and corrective action.
- Monitor and meet productivity standards.
- Implement discipline best practices.
- Support staff by identifying and utilizing resources.
- Promote professional development of Case Management Team.
- Hold regular staff meetings encouraging staff to share and participate in organizational and department quality improvement.
- Ensure staff participation in daily multidisciplinary rounds to discuss transition of care and any barriers to the discharge plan.
- Responsible for length of stay, avoidable days, prevention of readmissions, and discharge planning metrics.
- Participate in weekly complex case review.
- Record and report indigent fund spending.
- Implement appropriate monitoring techniques to ensure case management documentation is completed per department guidelines.
- Assure timely, effective discharge planning that involves care team, payor, along with patient and their families.
- Advocate and escalate any care delays.
- Participate in quality-of-care initiatives; reporting quality concerns to appropriate individuals, departments, and/or committees in a timely manner.
- Assist in the development and oversee implementation of care management policies and procedures.
- Collaborate with Social Work Manager to ensure effective collaboration with Social Work team.
- Collaborate with utilization management team to ensure timely delivery of regulatory forms, appropriate level of care, long stay outliers, and patients exceeding geometric length of stay.
- Minimum of BSN required; MSN preferred.
- Minimum of 2 years of supervisory/lead/manager level of experience in hospital Case Management is required.
- Licensed as a Registered Nurse (RN) in the state of Florida or appropriate compact licensure.
- Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
- Case Management Certification preferred.
- Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
