Job Title: Empowerment Case Manager

Job Summary:

The Empowerment Case Manager will be responsible for implementing the Empowerment and Direct Assistance Program, including client selection, enrollment, follow-up, and outcome measures. This role requires strong communication and interpersonal skills, as well as the ability to work with diverse populations.

Key Responsibilities:

  • Develop and maintain partnerships with community agencies
  • Meet with clients to determine eligibility and provide orientation
  • Conduct needs assessments and create individualized service plans
  • Provide ongoing case management and follow-up
  • Refer clients to outside resources when needed
  • Input client information into Client Track and maintain accurate documentation
  • Evaluate client success and advocate on their behalf
  • Complete timely and accurate reports

Requirements:

  • Bilingual (English/Spanish)
  • Minimum of a two-year college degree in human services or related field
  • 2 years of experience in human services or related field
  • Experience in record-keeping and data entry
  • Strong organizational and communication skills
  • Ability to work with diverse populations
