POSITION OVERVIEW

The Tennessee Community Services Agency specializes in providing Independent Support Coordination (ISC) services for the Department of Disability and Aging (DDA). Our ISC Program is dedicated to empowering individuals with intellectual and developmental disabilities to achieve greater independence within their communities through tailored community-based services. The primary focus of this role will be on clients participating in the TN Strong Families program (TNSF).

KEY RESPONSIBILITIES

  • Deliver case management services to individuals with developmental and intellectual disabilities.
  • Facilitate meetings, develop individual support plans, and submit these plans to DDA to initiate service requests.
  • Conduct monthly evaluations of services and ensure completion of essential documentation.

ESSENTIAL QUALIFICATIONS

  • A Bachelor's degree in social work, psychology, sociology, or a related discipline is preferred.
  • One to two years of relevant professional experience.
  • Experience with ISC or DDA, along with relevant certification, is advantageous. Familiarity with the Katie Beckett Waiver and TN Strong Families Grant Program is also beneficial.
  • Strong written and verbal communication skills are essential.
  • A valid driver's license is mandatory. Candidates must have reliable transportation and current automobile insurance.
  • Applicants must consent to the release of any investigative and criminal records obtained by the Federal Bureau of Investigation and the Tennessee Bureau of Investigation to the Tennessee Community Services Agency.
