Job Title: Home Health Registered Nurse RN Full Time

Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse RN Full Time. As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities:

  • Develop and implement individualized care plans in collaboration with the patient's physician and other healthcare professionals.
  • Provide skilled nursing care to patients in their homes, including wound care, infusion therapy, and catheter care.
  • Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
  • Collaborate with other healthcare professionals, including social workers, therapists, and pharmacists, to ensure comprehensive care.
  • Provide education and support to patients and their families on disease management, medication adherence, and other health-related topics.

Requirements:

  • Active RN license in the state of application.
  • 1 year of experience in a healthcare setting, preferably in home health care.
  • Basic understanding of Oasis and other electronic medical records systems.
