Home Health Physical Therapist

At CenterWell Home Health, we are dedicated to providing exceptional care to our patients in the comfort of their own homes. As a Home Health Physical Therapist, you will play a vital role in helping our patients achieve their maximum potential and maintain their independence.
Responsibilities:- Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses, and functional disabilities.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and record findings to develop or pursue treatment programs.
- Develop and implement a conditioning and rehabilitation program consistent with the physician's Plan of Treatment and the overall goals of the patient and rehabilitation team.
- Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
- Provide Physical Therapy Assistants and Home Health Aide staff with written instructions and care plans that reflect the current plan of care as related to therapy, supervise, and evaluate staff performance.
- Accurately and promptly document patients' care observations, interventions, and evaluations.
- Degree from an accredited Physical Therapy Program (approved by the APTA).
- Minimum of one year physical therapy experience preferred.
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Strong organizational and communication skills.
CenterWell Home Health offers a competitive salary range of $84,600 - $116,300 per year, as well as a comprehensive benefits package, including paid time off, 401(k) retirement savings plan, employee assistance program, business travel, and accident insurance.
