Job Title: Home Health Registered Nurse RN Full Time

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse RN Full Time. As a member of our clinical team, you will provide high-quality patient care in the comfort of their own homes.

Job Summary:

The Registered Nurse is responsible for delivering skilled nursing care to patients in their homes, adhering to confidentiality standards and professional boundaries at all times. This role requires a strong understanding of nursing principles, excellent communication skills, and the ability to work independently.

Key Responsibilities:
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address them.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress, identify areas for improvement, and adjust care plans as needed.
Requirements:
  • Active RN license in the state of application.
  • 1 year of experience in a healthcare setting, preferably in home health or skilled nursing.
  • Basic understanding of Oasis and Medicare Skilled Nursing experience preferred.
