Job Title: Home Health Licensed Practical Nurse LPN PRN

At Aveanna Healthcare LLC, we are seeking a skilled and compassionate Home Health Licensed Practical Nurse (LPN) to join our team. As a Home Health LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Job Summary:

The Home Health LPN is responsible for delivering skilled nursing care to patients in their homes, under the direction of a Registered Nurse. This includes assessing patient needs, developing and implementing care plans, and providing education to patients and their families.

Responsibilities:
  • Deliver quality patient care in compliance with physician orders and under the direction of a Registered Nurse.
  • Perform the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers.
  • Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk.
  • Educate clients and their family members based on client's specific needs.
  • Provide care in patients' homes using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes, etc.
Requirements:
  • An active LPN License in the state of application.
  • Valid CPR certification.
Preferred Qualifications:
  • Medicare Skilled Nursing experience.
  • Basic understanding of Oasis.
  • 1-year LPN experience in a home health or healthcare setting.
  • Home Care Home Base experience a plus.
