Job Summary:

The Registered Nurse is responsible for delivering skilled nursing care to patients in their homes, adhering to confidentiality standards and professional boundaries at all times. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Collaborate with healthcare teams to ensure seamless care transitions and coordination.
  • Document patient progress and outcomes in accordance with regulatory requirements and company policies.
Requirements:
  • Active RN license in the state of application.
  • 1 year of experience in a healthcare setting, preferably in home health or skilled nursing.
  • Basic understanding of Oasis and other relevant healthcare software.
