Job Summary:

We are seeking a skilled Licensed Practical Nurse to join our team as a Community Health Specialist. The successful candidate will be responsible for providing health care and wellness services to our consumers with intellectual disabilities in their community homes.

Key Responsibilities:

  • Provide health care and wellness services, including medication education, group psycho-education, and therapeutic interventions, to help individuals achieve their recovery goals.
  • Oversee the process of consumer medication administration, maintain close communication with the pharmacy, and review Medication Administration Records (MARs) for accuracy and completeness.
  • Regularly review and recommend changes to unit health and safety procedures and practices, including accident prevention, safety education, emergency procedures, medication administration, and personal care of consumers.
  • Provide assistance to residents who are unable to self-administer medication, supervise self-administered medication when indicated, and inject medication when prescribed.
  • Participate in multi-disciplinary treatment team meetings and Quality Improvement Program as assigned.
  • Timely report all crises and unusual incidents to the Nurse Manager and LTSR Director, and immediately report psychiatric and medical crises to the Psychiatrist.
  • Comply with all internal and external requirements and regulations regarding record keeping and recording.
  • Observe Universal Precautions in administering medications, disposing of needles, and handling waste products, etc.
  • Provide direct service to residents, including assistance with personal hygiene, bathing/showering, and all other tasks of daily living as needed.
  • Lead activities with residents, including educational, social, and leisure programs, and participate in community integration activities as needed.
  • Regularly document in clinical charts and utilize other designated forms.
  • Attend all meetings and training sessions as assigned.
  • Report all non-emergent shift and shift personnel problems to the Nurse Manager and/or LTSR Director in a timely and accurate manner.
  • In the event of a staff call-out, work to secure appropriate coverage via existing team members or other contracted/approved temporary staff organizations.
  • Comply with all Comhar Inc. policies and procedures with no unauthorized exceptions.

Requirements:

  • LPN + at least 2 years MH experience preferred.
  • Must have current nursing license required.
  • Valid driver's license required.
  • Must have own transportation.
  • ACT 31 required.
  • Position requires occasional lifting, transferring of individuals with an average weight of 150 pounds.

Skills Required:

Must have excellent verbal/written communication skills and work well with people; good, creative problem-solving skills; excellent clinical nursing skills, including assessment, knowledge of behavioral health systems, community resources; ability to work independently, and be flexible/adaptive in handling changing priorities in a fast-paced work environment; computer skills required.
