To excel in this position, you should possess a valid Practical Nursing license in Pennsylvania.

Key Responsibilities:
• Accurately document patients' medical histories

• Measure and record vital signs such as blood pressure, temperature, and heart rate

• Monitor patients' treatment progress and identify any side effects from medications

• Keep track of patients' conditions, including fluid intake and output, and maintain patient charts

• Assist patients with daily living activities, including eating and bathing

• Administer prescribed medications, injections, and basic medical care tasks

• Collect lab samples as required

• Provide emotional and psychological support to patients

• Communicate effectively with patients' families to offer guidance and comfort

Qualifications:
• Current LPN LICENSE in the state of Pennsylvania

• Successful completion of an accredited LPN program

• Experience in Long-Term/Skilled Nursing Facilities

• Familiarity with PCC systems

• Compassionate demeanor and ability to manage stress

• Strong team player skills
