Job Summary:
A highly skilled and compassionate Registered Nurse is needed to provide exceptional care to patients in their homes. This role involves assessing the patient's condition, developing personalized care plans, and coordinating with healthcare teams to ensure the best possible outcomes.

Key Responsibilities:
* Assess patient needs and develop effective care plans
* Coordinate care with healthcare teams and other stakeholders
* Provide high-quality nursing care to patients in their homes
* Monitor patient progress and adjust care plans as needed

Requirements:
* Graduate from an accredited school of nursing
* Active RN license in the Commonwealth of Kentucky (or other eNLC state with multistate privileges)
* Minimum 2 years clinical nursing experience
* Current driver's license and daily access to a reliable vehicle

Preferred Qualifications:
* Bachelor's Degree in Nursing
* American Heart Association Basic Life Support (BLS) certification
