Job Summary:

We are seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator. This role will involve coordinating the care of residents from admission to discharge, considering their individual needs and preferences.

About the Role:

The successful candidate will be responsible for assessing, planning, and implementing patient care plans in collaboration with other healthcare professionals. They will also provide education and support to patients and their families, and ensure that all aspects of patient care are met.

Key Responsibilities:

  • Coordinate resident care from admission to discharge
  • Assess and plan patient care in collaboration with other healthcare professionals
  • Provide education and support to patients and their families
  • Ensure all aspects of patient care are met

Requirements:

  • Degree in Nursing from an accredited institution
  • Current state licensure and at least 1 year of hospital experience within the last 2 years
  • Completion of 12 Continuing Education Units (CEUs) annually
