Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team. As a Home Health Physical Therapist, you will be responsible for providing high-quality physical therapy services to clients in their homes, promoting independence and improving their overall quality of life.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' physical abilities and identify areas for improvement.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
  • Continuously assess and revise care plans to ensure optimal outcomes.
  • Collaborate with nursing staff to develop and implement multidisciplinary care plans.
  • Provide education and instruction to clients and their families on rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations in client care management and services provided.
Qualifications:
  • Current MD Physical Therapist license.
  • Minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
